You are invited to complete this voluntary survey for the Downeast Community Population Health Initiative (DCPHI) by Dante Zanoni, program manager of The Connection Initiative. The Connection Initiative is a program of the Community Caring Collaborative.
The purpose of this survey is to learn about health issues that concern older adults and their caregivers in order to identify a health focus area for an emerging initiative that aims to improve health outcomes for older adults and tribal elders in Washington County and Passamaquoddy communities.
This survey is intended for Washington County community members over the age of 55 and for people caring for older adults in professional and non-professional roles. It should take about 7-10 minutes to complete. Other than the time it takes to complete it, there are no risks involved in doing this survey. You can skip any questions and stop the survey at any time. Your answers are anonymous.
