Goals of Care for Advanced Serious Illnesses Conversation – the Maine Hospice Counsel is offering a Physician Order for Life Sustaining Treatment (POLST) model training for medical providers, caregivers, clergy, hospice volunteers, and behavioral health counselors at the Calais Regional Hospital, 8:30 am to 4:30 pm.  This event is supported by the Washington County Thriving in Place Initiative, funded by Maine Health Access Foundation, and the CCC.  Visit this event site to register.  Registration must be completed by, Friday, September 22, 2017.  Scholarships available – check the applicable box.
